Hospital-based traumatic occupational injuries in Iran: Insights from a large national trauma registry (2017-2023)

Background: Occupational injuries represent a significant public health issue worldwide, with developing countries facing unique challenges in addressing their prevalence and outcomes. In Iran, these injuries contribute notably to mortality and morbidity; however, comprehensive epidemiological data remain scarce, particularly for occupational groups that account for a substantial proportion of hospitalized work-related injuries. Objectives: This study aimed to examine the frequency and outcomes of traumatic occupational injuries using data from the National Trauma Registry of Iran (NTRI), with an emphasis on their public health impact. Methods: In this registry-based study, we analyzed NTRI data from patients injured during income-generating work between August 2017 and November 2023. Demographics, injury patterns, amputations, fractures, and in-hospital outcomes were evaluated across six trauma centers. Results: Of 46,680 registered patients, 4,500 (9.68%) were included. Among them, 2,423 (53.84%) were laborers, followed by 986 (21.91%) self-employed, 919 (20.42%) technical workers, 131 (2.91%) employees, and 41 (0.91%) employers. The mean age was 38.2 ± 13.1 years. The mean Injury Severity Score (ISS) was 3.9 ± 4.2. Falls were the most common injury mechanism, accounting for 1,298 (28.9%) cases. Amputations and fractures occurred in 293 (6.5%) and 2,175 (48.3%) patients, respectively. Fractures most frequently involved the tibia/fibula (381 patients). Fracture patterns differed significantly across occupational groups (p < 0.001). Overall, 313 (7.0%) patients were admitted to the intensive care unit (ICU), and 32 (0.7%) died during hospitalization. In logistic regression analyses, technical workers had significantly lower odds of ICU admission than laborers in both crude (OR = 0.55; 95% CI: 0.38 to 0.79; p = 0.001) and adjusted models (adjusted OR: 0.58; 95% CI: 0.38 to 0.89; p = 0.013). Conclusion: This study highlights workplace safety gaps in Iran, particularly for occupational groups that account for a large share of hospitalized injuries, underscoring the need for improved training, safety protocols, and targeted preventive strategies .
