Predictors of hospital admission among patients with cutaneous leishmaniasis in an endemic focus in southern Iran: a retrospective study

Background: Leishmaniasis is an infectious disease that presents with various clinical manifestations. Due to resistance to standard treatments, some patients may require hospitalization for more effective and aggressive therapy. The objective of this study was to evaluate the predictors of hospital admission among patients with cutaneous leishmaniasis.Methods: This retrospective cross-sectional study was conducted among 328 patients admitted to Shahid Faghihi Hospital for cutaneous leishmaniasis between 2009 and 2019. The data were analyzed statistically and presented using bar charts and tables.Results: Out of 328 participants, males were affected almost as frequently as females. Among the patients, 135 (41.1%) had one lesion; of these, 49.3% had a lesion in the lower extremity, while 26.7% and 24% had lesions in the upper extremity and face, respectively. The average age of the participants was 50.6 ± 17.8 years, with 76% of the patients being over 40 years old. Additionally, 25.3% of the patients had diabetes, and 24 participants suffered from immunodeficiency, including 11 patients with AIDS, while the others were on immunosuppressive drugs. Positive ciultures were found in 48.5% of the patients, with 81.1% of these cultures identified as Staphylococcus aureus. Furthermore, 57.3% of the Staphylococcus aureus samples were resistant to methicillin.Conclusion: The majority of hospitalized patients exhibited similar clinical and laboratory characteristics, including being over 40 years of age, having lesions on the lower extremities, not receiving glucantim as treatment, having diabetes, and testing positive for Staphylococcus aureus. Each of these factors may be associated with a higher rate of admission among patients with cutaneous leishmaniasis.
