Efficacy of Novel MaxGym in Maxillary Protraction and Arch Expansion for Class III Correction

Aim: This study aimed to assess the efficacy of MaxGym therapy in promoting maxillary protraction and transverse arch expansion in patients with Class III malocclusion associated with maxillary deficiency.Methods: A prospective clinical trial was conducted on 14 growing patients (mean age: 10.9 years) requiring orthopedic maxillary advancement. The patented MaxGym appliance applied controlled orthopedic forces through a wall-mounted pulley system with a weighted resistance mechanism. Pre- and post-treatment assessments included lateral cephalograms, dental study models, and soft tissue analyses. Changes in skeletal, dental, and soft tissue parameters were evaluated using paired t-tests and Wilcoxon signed-rank tests (P < 0.05 considered significant) for non-normally distributed data. Bonferroni correction was applied for multiple comparisons.Results: Treatment resulted in significant skeletal and dental improvements. The SNA angle increased by 3.30° (P = 0.001), confirming forward displacement of the maxilla. Overjet improved by 3.80 mm (P = 0.002), indicating effective correction of anterior crossbite. A 1.85° increase in the FMA angle (P = 0.045) suggested mild posterior mandibular rotation. Significant transverse expansion of the maxillary arch was achieved, with intercanine, interpremolar, and intermolar widths increasing by 2.53 mm, 3.25 mm, and 2.75 mm, respectively (all P < 0.05).Conclusion: MaxGym therapy represents an effective, patient-controlled orthopedic modality for maxillary protraction and arch expansion, producing notable skeletal and dental changes. Its ability to achieve stable maxillary advancement with minimal dental compensation supports its potential as a comfortable and efficient alternative to conventional facemask therapy in growing patients with Class III malocclusion.
