The Effect of Calcium Metabolism Disorders in Patients with COVID-19 on Short and Long-Term Outcomes

Background and Objectives: Calcium metabolism disorders, including hypocalcemia and vitamin D deficiency, have been associated with disease severity in COVID-19 patients. These imbalances may contribute to prolonged hospitalization, ICU admission, and post-COVID complications. However, the exact impact of these disorders on the short- and long-term outcomes of COVID-19 patients has not been fully elucidated. This study aimed to investigate the impact of calcium metabolism disorders on short and long-term outcomes in patients with COVID-19. Methods: In this cross-sectional-analytical study in 2021 in Afzalipur Medical Center of Iran, Covid-19 patients between 18 and 60 years old were investigated. Blood samples were collected for analysis of factors including calcium and vitamin D levels. Demographic data, clinical status and laboratory results were collected. Statistical analysis utilized descriptive measures and inferential tests, including Chi-squared and logistic regression, to identify outcome predictors. Analyses were performed using SPSS v21. Results: In this study involving 162 participants, 7% required mechanical ventilation, and 18% faced mortality during hospitalization or within three months post-discharge. Additionally, 54% of patients reported PCS symptoms up to three months following discharge. Although no significant association was observed between mortality and calcium metabolism disorders, hypoparathyroidism and vitamin D levels were significantly associated with intensive care unit admission. Furthermore, significant associations were identified between C-reactive protein status and conditions such as hyperparathyroidism (P=0.01), hypoparathyroidism (P=0.01), adjusted hypercalcemia (P=0.008), adjusted hypocalcemia (P=0.01), and vitamin D levels (P=0.03). Age, hyperparathyroidism, and vitamin D were also significantly associated with PCS. Conclusion: In COVID-19 patients, our research suggests possible calcium disorders, especially hypocalcemia, that could contribute to negative outcomes like intensive care units (ICU) admission, mechanical ventilation, elevated CRP levels, and long-term issues like PCS. While fatigue and neurological symptoms are common in PCS, they are diverse and unpredictable, possibly linked to calcium imbalances.
