Correlation between Retinopathy and Severity of Coronary Artery Disease in Type II Diabetic Patients with Acute Coronary Syndrome: A Cross-Sectional Study

Background and Objectives: The duration of type II diabetes mellitus (T2DM) and the severity of hyperglycemia are among the effective and consistent risk factors for the development and progression of microvascular diabetic complications. This study aimed to explore the correlation between diabetic retinopathy as an indicator of microvascular damage and the severity of coronary artery disease (CAD) in diabetic patients.   Methods: We enrolled 60 consecutive patients with type II diabetes who were candidates for CAD diagnosis and were referred for coronary angiography from February 2019 to September 2020. Patients were referred for retinal evaluation and visual acuity assessment within one month after coronary angiography.   Results: The duration of diabetes had a negative correlation with the retinal thickness (P = 0.02) and peripapillary thickness (P < 0.001). Inside vessel density was found to have a negative correlation with HbA1c (P = 0.04). There was also a negative correlation between the vessel score and the retinal superficial layer vessel density (r = -0.322, P = 0.013). Gensini score significantly correlated with HbA1c (P = 0.04) and serum triglyceride level (P = 0.01).   Conclusion: There was a significant negative correlation between coronary vessel score and retinal vessel density in the superficial layer as an indicative parameter of retinopathy. For diabetic patients, it is mandatory to develop diagnostic systems and risk-assessing criteria to evaluate the odds of developing retinopathy.
