Background and Objectives: Superior mesenteric vein thrombosis (SMVT) is a rare and high-risk condition, but the medical community remains divided on the treatment of this condition. The objective of this study was to systematically evaluate the clinical efficacy and safety of conservative versus surgical treatment of superior mesenteric vein thrombosis. Methods: Observational studies published between January 2011 and December 2021 that compared outcomes of conservative and surgical treatments for SMVT were systematically searched using both computer-based and manual methods. The literature was screened according to pre-determined inclusion and exclusion criteria. The Newcastle-Ottawa Scale (NOS) was used to assess the risk of bias in the included studies. Data were extracted for meta-analysis or descriptive statistics. Results: A total of sixteen observational studies were included, with NOS scores above seven stars. Results showed that conservative treatment was superior to surgical treatment in patients with SMVT [RR=1.06, 95%CI (1.01–1.11), Z=2.16, P=0.03]. Conclusion: For most patients with SMVT, conservative treatment is more effective than surgical intervention, offering less trauma, lower treatment requirements, and a higher survival rate, and can therefore be considered as the first-line treatment.
