Superior Vena Cava Syndrome as an Initial Presentation of Anaplastic Thyroid Carcinoma: A Case Report and Literature Review

Anaplastic thyroid cancer (ATC) is a pathological type of thyroid cancer (TC) with a low incidence. Intravenous tumor thrombus caused by ATC is extremely rare and, to our knowledge, has occasionally been reported. Among them, manifestations of superior vena cava syndrome (SVCS) are even rarer. We describe an Asian patient with anaplastic thyroid carcinoma who presented with an intravenous tumor thrombus combined with SVCS. The patient was a 78-year-old female who presented with painless swelling of the face and neck for more than one month. An ultrasound of the neck showed a hypoechoic mass in the right thyroid lobe. Color Doppler ultrasound of the cervical vessels showed thrombotic obstruction within the right internal jugular and superior thyroid veins. Contrast-enhanced ultrasonography revealed tumor thrombus. Fine needle aspiration biopsy confirmed the right thyroid nodule as ATC. We also reviewed the literature on intravenous tumor thrombus caused by TC to further characterize this rare disease.
