Ten Years Investigation of EGFR Mutation Screening in Non -Small Cell Lung Cancer and Comparison of Real-time PCR and Reverse Strip Assay Methods

Background and Objectives: Non-Small Cell Lung Cancer (NSCLC) is one of the increasing cancers. Many studies have reported EGFR mutation as one of the main factors in NSCLC pathogenesis. Using Reverse Strip Assay (RSA) to detect EGFR mutation is a common method. Cell free DNA (cfDNA) and biopsy samples are used to detect EGFR mutation. In this study, we evaluated the sensitivity of these two samples to detect EGFR mutation. Methods: A total of 306 (from 2012-2022) patients with NSCLC participated in the study; biopsy samples were obtained from all patients. Among them 20 patients who had EGFR gene mutations were followed for 6 months using cfDNA by RSA and RT-PCR methods. All data were expressed as frequency (percentage). Analyzes were performed using SPSS (version 23) software. Results: The EGFRRSA analysis from biopsy samples revealed that 82 (26.8%) of 306 FFPE samples carry an EGFR mutation, of which the most prevalent was p.L858R(c.2573T>G) in exon 21 (n = 18 (5.9%)), p.E746-A750del (c.2235-2249del) (n = 16 (5.2%)) in exon 19 and p.E746-A750del (c.2235-2250del) (n = 12 (3.9%)) in exon 19 mutations. No mutation was detected for 208 (70.2%) FFPE samples. Twenty out of 306 patients were randomly selected and followed up for 6 months; instead of biopsy samples, cfDNA was used to evaluate EGFR mutations in these 20 patients. Also, mutations were re-checked by RT-PCR in parallel. The results showed that cfDNA Strip Assay and RT-PCR methods had the same sensitivity with cfDNA samples instead of biopsy. Conclusion: cfDNAs instead of biopsy samples are more suitable for EGFR mutation detection using strip assay. cfDNA is more convenient, non-invasive, and economical.
