Surgical Techniques for Wassel Type IV Preaxial Polydactyly of the Hand: A Case Series and Literature Review

چکیده

Polydactyly of the hand is a common congenital upper limb anomaly encountered by pediatricians and hand surgeons. Preaxial (radial) polydactyly is relatively common; the treatment is critical, and its reconstruction is more complex than ulnar polydactyly. This study aims to report and review surgical treatment of preaxial polydactyly cases. Three patients were selected with different surgical ages (ten months, six years, and two years) and genders (two males and one female). All patients were classified as Wassel type IV and underwent racquet incision and radial thumb amputation. Postoperative immobilization was performed in two patients, while the other was not immobilized. A postoperative complication was noted in one patient as a zigzag deformity. Thumb reconstruction, as the treatment for preaxial polydactyly, is not simply a matter of excising the smaller digit; it requires attention to the bones, ligaments, tendons, muscles, and neurovascular structures. In conclusion, the proper age for surgery, Wassel type classification, incision design, postoperative immobilization, and surgical technique procedures yield optimal outcomes and may reduce postoperative complications.
