Comparison of the effect of oral synbiotic supplementcompared with placebo in the treatment of plaque typepsoriasis: a randomized, double-blind, clinical trial

Background: Recent studies suggest a potential role for the gut microbiome in modulating immune responses relevant to psoriasis. This study evaluates the efficacy of probiotics as an adjunctive treatment in the management of psoriasis.Methods: Fifty patients with psoriasis completed this randomized, double-blinded, placebo-controlled trial. They were treated once with daily probiotic or placebo capsules for three months. Key outcomes-the Psoriasis Area and Severity Index (PASI) and static Physician Global Assessment (sPGA) scores- were measured at baseline and monthly throughout the study. Quality of life was also assessed before and after the treatment period.Results: Repeated measures analysis of variance examining the effects of time, treatment and their interaction on PASI scores showed significant improvement over treatment sessions in both the probiotic and placebo groups (P < 0.001). However, no significant difference was observed between the groups (P = 0.809). Static PGA scores decreased significantly after 12 weeks of treatment in both the probiotic and placebo groups (P = 0.001 and P < 0.01, respectively). Nevertheless, sPGA values at the end of treatment did not differ significantly between groups (P = 0.700). Improvement in DLQI scores did not differbetween treatment groups (P = 0.275), and a significant improvement in quality of life was observed only in the placebo group at the end of the study (P = 0.031).Conclusion: Probiotic supplementation is effective in improving clinical outcomes (PASI and sPGA) in plaque-type psoriasis, however this effect was not superior to that of placebo.
