Serum anti-tissue transglutaminase-2 antibodies (IgA) inpatients with chronic plaque psoriasis

Background: Measurement of antitissue transglutaminase-IgA is the standard initial test for diagnosing celiac disease. However, in certain cases, additional tests such as antitissue transglutaminase-IgG, anti endomysial antibody-IgA, antigliadin antibody-IgA, AGA-IgG, and deamidated gliadin peptide antibodies (DGP-IgA, DGP-IgG) may be utilized for diagnosis and screening. This study was conducted to evaluate the prevalence of serum antitissue transglutaminase-IgA in patients with chronic plaque psoriasis.Methods: This case-control study included 68 psoriasis patients. They were divided into two equal groups: Group 1 (study group) comprising patients with chronic plaque psoriasis, and Group 2 (control group, n = 34) consisting of healthy individuals. All participants underwent comprehensive history taking, clinical examinations, and laboratory investigations.Results: A positive correlation was observed between the Psoriasis Area and Severity Index (PASI) score and antitissue transglutaminase-IgA levels (r = 0.518, P Value = 0.002). Antitissue transglutaminase-IgA significantly predicted patients with chronic plaque psoriasis (P Value < 0.001 with an area under the curve (AUC) of 0.914. At a cut-off value > 13, the test demonstrated 88.24% sensitivity, 85.29% specificity, an 85.7% positive predictive value and an 87.9% negative predictive value.Conclusion: Chronic plaque psoriasis is associated with elevated levels of antitissue transglutaminase-IgA. Additionally, antitissue transglutaminase-IgA levels correlated positively with the PASI score. This marker can predict chronic plaque psoriasis at a cutoff > 13, with a sensitivity of 88.24% and a specificity of 85.29%.
