The clinical overlap between inflammatory bowel disease (IBD) and irritable bowel syndrome (IBS) presents diagnostic and therapeutic challenges, despite their traditionally distinct categorizations as organic and functional gastrointestinal disorders. This review examines the convergence of symptoms and pathophysiological features in IBD and IBS, highlighting mechanisms, including inflammation, microbial dysbiosis, and brain-gut axis dysfunction. Emerging evidence suggests that a significant proportion of IBD patients in remission exhibit IBS-like symptoms, complicating diagnosis and management. Genetic and proteomic analyses reveal minimal overlap, supporting the distinction between the two conditions. This paper reviews recent genetic data, immunological profiles, and brain-gut axis findings to reassess the connections between IBD and IBS, emphasizing the need for nuanced clinical evaluation to prevent misdiagnosis and ensure appropriate care.
