Identification of Root Causes of an Accident that Occurred During Mining Operations: A Case Study in Russia

This article analyzes the level of occupational injuries, on the basis of which a vector for the development of the accident investigation process at mining facilities is proposed. The paper assesses the need to introduce methods for analyzing the root causes of in the investigation process in order to improve the Occupational Health and Safety Management System and achieve the goal of «Zero injuries» for mining workers. Based on the application of root cause analysis methods, a timeline of fatal accidents that occurred during mining operations was constructed, which made it possible to determine the cause-and-effect relationships of workers' actions, conditions and critical events. As a result of the application of the «Five Whys» method, the root causes of the accident have been identified. The complex application of the methods proposed in the work shows the whole picture of the accident due to visualization and an accurate logical sequence of events. Employees and responsible persons have been identified who violated the requirements of regulatory and local documents in the field of labor protection, industrial safety and rules for safe work. The results of the study are presented in graphical form in order to visualize all the structures of the Occupational Health and Safety Management System and assess the need to improve the process of investigating the accident in production. The conducted research shows the effectiveness of integrating the process of analyzing the root causes of the accident by better understanding the actions and conditions of employees and responsible persons, followed by the development of innovative corrective measures.
