Heterotopic pregnancy is the simultaneous presence of pregnancies at distinctly different implantation sites. Once the ectopic pregnancy is located in the interstitial segment of the fallopian tube, it is referred to as interstitial pregnancy. The management of heterotopic pregnancy is intricate due to its nonspecific presentation and diagnostic challenges. This case report describes a 33-year-old woman who had conceived pregnancy via Assisted Reproductive Technology due to infertility. Her symptoms were mild abdominal pain and spotting for 1-2 weeks. An intrauterine pregnancy was confirmed in conjunction with an interstitial ectopic pregnancy following transvaginal ultrasound. She underwent laparoscopic treatment and hysteroscopy. This case underscores the significance of early detection and appropriate management of heterotopic pregnancy. A history of In Vitro Fertilization, accompanied by non-specific symptoms such as abdominal pain, should raise suspicion for heterotopic pregnancy, and that a minimally invasive approach can be feasible for treatment of an heterotopic pregnancy.
