Possible Predictors of Successful Intrauterine Insemination in Iraqi Subfertile Couples: An Observational Retrospective Study

Background & Objective: Intrauterine insemination, a procedure that entails the direct insertion of processed and concentrated spermatozoa into the uterine cavity coinciding with the ovulatory phase, is widely employed as a preliminary therapeutic intervention for a variety of infertility etiologies. The present study was conducted with aim to identify the potential predictors of successful Intrauterine Insemination (IUI) in infertile couples.Materials & Methods: In this observational retrospective study, the records of infertile couples from January 2019 until January 2025 were retrieved from archival systems in these fertility centers (Fertility Center/Al-Sadr Medical City /Al-Najaf Governorat and Kamal Al-Samurai fertility Center in Bghdad). All available variables found in retrieved patients’ records were considered for statistical description and analysis. Markers of ovarian reserve, seminal fluid analysis, endometrial thickness, results of hysterosalpingography and hormonal assessments were the main variables obtained from lists of investigations.Results: The younger female, male age, and shorter duration of infertility were significantly associated with positive pregnancy tests (P<0.05), while female BMI was comparable between the groups (P=0.553).In addition, a higher mean of endometrial thickness was the sole significant predictor of a positive pregnancy test (P<0.001). Moreover, the post-wash total sperm progressive motility (TSPM %) emerged as the singular significant predictor of a positive pregnancy outcome.Conclusion: Couples with young age, shorter duration of infertility, adequately thick endometrium, secondary type of infertility and greater post-wash total sperm progressive motility are the best candidates for IUI as first line fertility treatment approach.
