SEM Analysis and Morphological Investigation of Efficacy of Smear Layer Removal by Ultrasonic versus Syringe Irrigation in Primary Canines

The efficacy of irrigation with syringe and ultrasonic techniques for the removal of intracanal stain layers from primary canine teeth was compared in this study using a variety of irrigants. In this in vitro experimental study, 42 primary canines were randomized into two groups: one that received irrigation with a syringe and the other that received irrigation with an ultrasonic technique. Each group was broken down into three subgroups: one received 0.9% saline, another was treated with a mix of 5.25% sodium hypochlorite and 17% ethylenediaminetetraacetic acid (EDTA), and the last group used 6% citric acid (n=7). The teeth were cut in half and examined with scanning electron microscopy (SEM) after they underwent root canal treatment and irrigation. To determine whether a stain layer was present in the coronal, middle, and apical sections of the roots, researchers employed the Kruskal-Wallis and Mann-Whitney tests. The ultrasonic technique demonstrated a significantly greater efficacy than the syringe in all three locations of the root in the citric acid subgroup (P<0.05).  In the other subgroups, there was no significant difference between the syringe and ultrasonic technique (P>0.05). The ultrasonic method using 5.25% sodium hypochlorite and 17% EDTA achieved optimal smear layer elimination, regardless of the root area, along with dentinal tubule degradation.  The effectiveness of smear layer removal was diminished in the apical area, regardless of the irrigation strategy and kind of irrigant used.
